Shalom Blessings Community Care is a NDIS registered support provider. Our team of registered nurses and disability support workers works with NDIS participants to achieve their goals by providing individually tailored person-centred support.
Nursing Support
Shalom Blessings Community Care’s registered nurses provide health and personal care support to NDIS participants in their homes or communities. Nursing support we provide include, but not limited to:
Medication management: safe administration of medications and injections, along with monitoring for side effects,
Diabetes management: blood glucose monitoring, insulin administration, and diabetes management education,
Continence and stoma care: assessment and management of continence needs, including catheter and stoma care,
Wound management: professional care for various wounds, including pressure injures, post-surgical wounds, and chronic ulcers.
Community Participation
Participating in social, recreational, and community activities help all of us feel more connected, independent, and lively. We provide all the support needed for NDIS participants to engage in activities that make them smile, from providing transport, assistance with accessing bathroom, emotional support, calendar management, to liaising with mainstream service providers.
Shalom Blessings Community Care also provides other NDIS support such as Supported Independent Living, Short Term Accommodation, and Support Coordination.
